Medical Plans Comtek employees become eligible for health insurance on their date of hire. There are two different health care plan options to choose from (HMO or PPO). Employees and Comtek share the cost of health care coverage. Dental Care Comtek employees become eligible for dental insurance on their date of hire. Comtek offers a PPO dental plan which allows employees to receive dental services from both Network or Non-Network providers. Employees and Comtek share the cost of dental coverage. Vision Program Comtek employees become eligible for vision insurance on their date of hire. There are two different dental plan options to choose from (Exam & Materials or Materials only). Gold’s Gym Comtek takes a personal interest in their employee’s health and wellness. Therefore, Comtek provides health club memberships to all employees at no cost. 401(k) Plan Employees who are 21 years of age or older may participate in the company’s 401(k) program on the first day of the month following their hire. Comtek matches 50 cents on the dollar up to a maximum of 3%, and full vesting is obtained after five years of continuous employment. Employee Assistance Program Comtek understand that employees face many challenges in life. Therefore, Comtek offers this confidential program, provided at no cost to assist employees and their immediate family members with personal concerns. Services include: |
Short Term Disability Comtek provides a voluntary short-term disability (STD) plan for employees who are unable to work because of a disability due to an injury or illness. STD benefit payments begin on the 8th day of a disability. The benefit equals 66 2/3% of the employee’s weekly earnings, up to a maximum of $2,500 per week. This benefit is reduced by other income sources. Long Term Disability Comtek employees become eligible for long-term disability (LTD) on their date of hire. Comtek provides this coverage at no cost to employees. LTD payment begins when an employee is out past 180 days. This benefit pays up to 60% of the employee’s monthly up to a maximum of $5,000. This amount is reduced by other income sources. earnings if they are out past 180 days. Life Insurance To protect employees and their family, Comtek provides employees 1 times their annual salary for basic life insurance and accidental death and dismemberment at no cost to employees. Employees may also elect to purchase additional coverage (1X, 2X, or 3X of their annual salary) for themselves, their spouses or children. Medical Reimbursement Account Eligible Comtek employees can arrange to have pre-tax money automatically deducted from their paychecks and placed into a health care reimbursement account. Being able to pay medical and dental bills, including co-pays and deductibles, with pre-tax money can save you money each year. Annual plan election is a maximum of $3,000. Educational Reimbursement Program Employees become eligible for this benefit after six months of continuous employment. Comtek reimburses employees 90% up to $3,000 per calendar year with a passing grade of “B” or better. |
